Medical Records Release
To obtain medical records from our institution or have medical records sent to another facility, please fill out the Authorization for Use and Disclosure of Information form linked below and turn it in to our front office. If you wish for records to be sent or requested, please note the fax number of the facility as this is how we will send the release of information request.
